Health insurance for students from outside the EU in Poland. How to sign an agreement with the NFZ in Poznań?
Health insurance for students from outside the EU in Poland. How to sign an agreement with the NFZ in Poznań?
This applies primarily to a student from outside the EU who lives in Poland and is not insured through work, as a family member, or under the healthcare system of another EU or EFTA country. Before signing an agreement, it is worth checking your individual situation: for example, employment under an employment contract provides a separate basis for health insurance. Starting university does not automatically mean that the university has registered you for insurance.
How much is the contribution?
According to NFZ information current as at 24 September 2026, the contribution for a foreign national who is a student from outside the EU is PLN 55.80 per month. This rate does not apply to everyone who signs a voluntary insurance agreement. For a person who does not qualify for a special contribution basis, the minimum contribution for July–September 2026 is PLN 845.67 per month. Before paying, check the current amount on the NFZ website and which category applies to you.
The contribution is monthly and cannot be reduced for part of a month: the full amount is payable even if the insurance was in force for only part of that month. Payment is due by the 20th day of the following month. It must be made to your individual ZUS contribution account.
Where can you arrange this in Poznań?
The NFZ indicates that voluntary health insurance in Poznań is handled at:
ul. Grunwaldzka 158, 60-309 Poznań
ul. Baraniaka 88d (Building E), 61-131 Poznań
What documents should you prepare?
The NFZ lists the following in particular:
A passport and a document confirming your legal stay in Poland, such as a visa or residence card.
A current certificate from your university confirming your student status. According to the NFZ, the certificate is valid for 30 days from its date of issue. It is also worth bringing your student ID card.
A document confirming that you live in Poland and the copies required under the NFZ instructions. These may be needed.
An application for voluntary health insurance with the accompanying declaration. You can prepare these on site.
If the student is under 18: written consent from a parent or legal guardian and the copy of their document specified by the NFZ.
The notification confirming the assignment of a PESEL number.
What should you do after signing the agreement?
Register with ZUS using form ZUS ZZA within 7 days of signing the agreement. You register when the insurance begins; you do not submit this form again every month simply because another contribution is due.
Submit a ZUS DRA declaration every month for the period during which you are insured and pay the contribution to your individual ZUS account. ZUS ZZA and ZUS DRA are different forms.
Maintain continuous insurance coverage. According to the NFZ, insurance ends, among other circumstances, after one month of continuous arrears in paying contributions or when the agreement is terminated. If you obtain another basis for compulsory health insurance, clarify the change with the NFZ and ZUS.
Family members: A foreign national who is voluntarily insured as a student cannot register family members for insurance under this agreement. Form ZUS ZCNA should therefore not be presented as a standard step for this group.
When can you start using healthcare services?
Voluntary insurance begins on the date specified in the agreement with the NFZ, but no earlier than the date on which the application is submitted. The agreement cannot be backdated. As a rule, entitlement to healthcare services ends 30 days after the agreement is terminated or expires. Keep your copy of the agreement, confirmation of your ZUS registration and proof of contribution payments.
